Glucosamine is an amino sugar that occurs naturally in the fluid surrounding joints and in cartilage, where it serves as a building block for glycosaminoglycans and proteoglycans that give cartilage its structure and shock-absorbing properties. Commercial supplements are most commonly derived from shellfish exoskeletons (glucosamine sulfate or hydrochloride) or produced synthetically for those with shellfish allergies. Decades of research into glucosamine for osteoarthritis have produced a genuinely mixed evidence picture: some large trials and meta-analyses report modest benefit, particularly for the sulfate form and particularly for stiffness rather than pain, while others, including large industry-independent trials, report effects no different from placebo.

Clinical Summary
A 2-year randomized trial (GAIT) and a subsequent network meta-analysis found glucosamine produced a statistically significant, though clinically modest, improvement in joint stiffness compared to placebo, with less consistent evidence for pain reduction. Industry-independent trials have generally reported smaller effects than commercially funded trials.
Dosage Guidelines
1500 mg/day, taken as a single dose or divided across the day, generally continued for at least 8-12 weeks before assessing effect.
Safety & Side Effects
Generally well tolerated; mild gastrointestinal upset has been reported. Shellfish-derived forms carry an allergy risk for those with shellfish allergies (synthetic and vegetarian forms are available).
Known Interactions
Theoretical mild interaction with blood-thinning medications (warfarin); caution advised for individuals with shellfish allergy when using shellfish-derived forms.
